Connect2Health mobilizes student health advocates to link underserved patients with social services, and prepares students to be empathetic future health professionals who understand the importance of social determinants of health.

Our Clinics


Fourth Street Clinic helps Utahns experiencing homelessness improve their health and quality of life by providing high-quality health care and support services. Connect2Health volunteers at this clinic aim to connect under-represented population to other local community resources through providing Resource Referrals. Volunteers initiate one-on-one interactions with patients in waiting room and work individually with them to come up with a tailored referral packet.

The Wellness Bus creates healthier communities by offering the following services to Utahns who do not have access to quality healthcare: 
    • Chronic Disease Screening
    • Nutrition Education
    • Health and Wellness Counseling
    • Referrals to Social Services 
As Connect2Health volunteers we cover the fourth service on the list by bridging the gap between social service providers and patients. We meet with our patients in a one-on-one setting and discuss their needs in order to create a list of resources relevant to their needs provided. 
The Wellness Bus aims to reduce the burden of diabetes and other chronic diseases in medically underserved areas by providing important preventative health services. 

 

  • Internal Medicine

Internal Medicine is a collaboration between all different levels of healthcare. We cover four units across two floors at the main University Hospital. We work closely with the case managers, social workers, and nurses to refer resources to our patients and ensure their social needs are met.

  • Mother Baby

Volunteers in the Mother Baby Unit meet with new mothers and families in the Maternal New Born Care Center, Women's Special Care, and Neonatal Intensive Care Unit to provide resources to meet their social needs. We most often help with finding resources for car seats, clothing, baby supplies, and diapers. We work closely with the social workers and case managers in these units to meet the needs of these patients.

  • SUPeRAD (Substance Use in Pregnancy Recovery Addiction Dependence) 

Volunteers in SUPeRAD work closely with Dr. Marcela Smid and clinical staff to meet with mothers that are dealing with substance abuse and addiction during pregnancy. This clinic helps mothers work on recovery and overcoming addiction by allowing them to meet with not only their OBGYN, but a psychologist, substance abuse recovery coach, social workers and group therapy, as well as the Connect2Health volunteers to help with resources.

  • Health Plans

University of Utah Health Plans serve members in the Mountain West specializing in the health plan administration of medical, behavioral health, and pharmacy benefits for fully-insured and self-funded employer groups, individuals and families, as well as Medicare and Medicaid. Connect2Health (C2H) Interns work closely with University of Utah Health Plan’s (UUHP) care management team to reach out to new members, conduct screenings, introduce care management program, complete the Health Risk Assessments and provide community resources for those needing additional support.
